Police of the Czech Republic – Filling in the application form
a foreigner may reside in the territory of the Czech Republic and prove his identity with a travel document (passport) or, if he is a citizen of the European Union, with an identity document (identification card – similar to the ID card of a citizen of the Czech Republic) or, if he has in one of the member states European Union permanent residence permit or long-term residence card Permit to stay for foreigners.
the foreigner is obliged to personally fill out and sign the application form [§ 103 letter b) Act No. 326/1999 Coll.]; the registration form can also be filled out and signed via, for example, a tablet.
if the foreigner refuses to fill out and sign the registration form, such an event will be reported to the locally competent foreign police department of the regional police headquarters (local jurisdiction is determined by the address of the accommodation facility),
if a child under the age of 18 does not have a travel document and if it is not entered in the parent’s travel document, such an event shall be reported to the locally competent foreign police department of the regional police headquarters (local jurisdiction is determined by the address of the accommodation facility),
the foreigner’s accommodation is announced, not the accommodation reservation.
